Drug candidate to be assessed for managing voluntary motor disturbances in Huntington disease

JERUSALEM–(BUSINESS WIRE)–Sep. 27, 2012– Teva Pharmaceutical Industries Ltd (NYSE: TEVA) announced today that it has concluded an Asset Transfer Agreement with NeuroSearch A/S of Denmark (OMX: NEUR) to purchase all rights, assets and obligations relating to Huntexil® (pridopidine / ACR16), a drug candidate being developed for the symptomatic treatment of hand movement, balance and gait disturbances in Huntington disease (HD). Under the agreement, Teva will pay to NeuroSearch approximately $26 million (DKK 150 million) over a period of at least six months. Regulatory and commercialization milestone payments may result in additional funding for NeuroSearch.

Previous trials in the US, EU and Canada demonstrated significant symptomatic relief for patients with HD including improved hand movements and improved gait and balance. These results were observed without any side effects such as sedation and depression seen with other therapies such as neuroleptics and tetrabenazine. Teva believes that Huntexil® will, used as a symptomatic agent, make a real difference to the quality of life for patients suffering from HD.

Huntexil® Development Program

Huntexil® (pridopidine / ACR16) is an oral small molecule dopamine D2 stabilizer being developed for the symptomatic treatment of non-choreic motor disorders, including for Huntington disease (HD). Huntington disease affects about one person in 10,000 in North America and Europe and generally results in death within 15 to 25 years of diagnosis.

Teva intends to design and complete new clinical studies of Huntexil® to assess its potential for symptomatic relief of HD. Advanced-stage clinical studies of Huntexil® conducted in the US, EU and Canada in patients with HD demonstrated a significant treatment effect on Total Motor Score [TMS]), but failed to meet the primary endpoint (Modified Total Motor score [mTMS]). Data from the clinical studies were presented to the FDA and EMA in the first half of 2011, but were found insufficient to file for marketing approval.

About Huntington disease

Huntington disease (HD),is a fatal neurodegenerative disease characterized by uncoordinated and uncontrollable movements, cognitive deterioration, and behavioral and/or psychological problems. The classic onset of HD symptoms typically occurs in middle age, but the disease also manifests in children and the elderly. Disease progression is characterized by a gradual decline in motor control, cognition, and mental stability and generally results in death within 15‐25 years of clinical diagnosis.

HD is a genetic disease, transmitted via autosomal‐dominant inheritance. The defective gene, found on chromosome 4, causes the production of a mutant protein, huntingtin, which aggregates in the central nervous system (CNS) and results in the pathogenesis of HD. The prevalence of HD is approximately 10 per 100,000 in the US and Europe. The only currently marketed product in the US indicated for HD is tetrabenazine, which has no effect on non‐choreic symptoms and disease progression, which remain as significant unmet medical needs; also, tetrabenazine is associated with serious side effects such as suicidality and depression.

I risultati dello studio clinico MermaiHD sull’Huntexil per i disturbii del movimento nella MH sono stati pubblicati sulla rivista scientifica Lancet Neurology. L’Huntexil e i disturbi del movimento



Huntexil è il nome commerciale dell’ACR16, noto anche come pridopidina. Sviluppato dalla Neurosearch, una società farmaceutica Danese, l’Huntexil è un nuovo possibile trattamento per migliorare i sintomi della malattia di Huntington.


L’obiettivo dell’Huntexil è il movimento, o sintomi “motori” della MH. A differenza dei farmaci esistenti, l’Huntexil non è solo mirato ad alleviare i movimenti involontari (‘còrea’ e ‘distonia’), ma a migliorare la funzionalità motoria nel suo complesso incluso l’equilibrio e il controllo volontario.

Neurosearch ha condotto due studi clinici sull’Huntexil — la sperimentazione MermaiHD in Europa e la sperimentazione HART negli Stati Uniti. I risultati di tali sperimentazioni sono stati presentati attraverso comunicati stampa e a diversi meeting scientifici su cui si è già scritto in passato su HDBuzz.

E ‘opportuno sottolineare che l’Huntexil è un farmaco per il controllo dei sintomi, enon abbiamo motivo di credere che sia in grado di modificare la malattia nella MH- non vi è alcun indizio che suggerisca sia in grado di prevenire o rallentare la progressione della malattia.

Perché in prima pagina?

L’Huntexil è tornato tra le notizie di questa settimana, con un insieme confuso di titoli che andavano da “la Pridopidina promette bene nella sperimentazione” a “Il farmaco delude nella malattia di Huntington”. Allora che cosa è realmente successo?

La risposta è semplice: non molto. Questi titoli non si riferiscono ad un nuovo processo, o a dati nuovi. Questi sono la conseguenza della pubblicazione ufficiale dei risultati dello studio MermaiHD sulla rivista scientifica peer-reviewed The Lancet Neurology.

Cosa dice l’articolo?

L’articolo sulla rivista Lancet Neurology dice più o meno quello che ci aspettavamo dalle precedenti presentazioni della Neurosearch riguardo ai dati dello studio MermaiHD. La conclusione è che il farmaco non è riuscito a soddisfare l’”obiettivo primario” — il quale rappresenta il criterio di riferimento per definirne il successo — che era stato fissato prima ancora della sperimentazione.

Questo fallimento è stato il motivo per cui la FDA in America, e l’EMA in Europa, hanno rifiutato entrambe di approvare l’Huntexil per l’uso nei pazienti, insistendo sul fatto che un’ulteriore sperimentazione che permetta di raggiungere l’obiettivo primario è necessaria prima di utilizzarlo nei pazienti.

Tra le tante cose diverse che sono state misurate, c’erano un paio di risultati in base ai quali la pridopidina potrebbe ancora avere alcuni benefici. Al livello più alto delle due dosi testate, alcuni risultati sul movimento sembravano essere leggermente migliori nei pazienti trattati. E il farmaco è risultato essere abbastanza sicuro senza molti effetti collaterali. Sulla base di tali risultati, Neurosearch sta esercitando pressioni per effettuare una sperimentazione ulteriore.

Perché c’è confusione?

La confusione nei titoli dei giornali riguardo questo articolo mette in luce un problema comune nella scienza — la tendenza delle fonti di informazione di voler proporre un messaggio molto semplice, quando la scienza è raramente semplice.

Come sempre, incoraggiamo i nostri lettori ad andare oltre ai titoli e non affidarsi ad un’unica fonte di informazione sulle novità. Il nostro articolo “dieci regole d’oro” fornisce consigli per trovare la verità tra la confusione.

E adesso?

Quindi, fondamentalmente la situazione non è realmente cambiata per l’Huntexil. Se non altro, l’articolo di Lancet Neurology rassicura che la sperimentazione MermaiHD è stata ben gestita, il farmaco resta interessante ed è ben tollerato.

Ma una sperimentazione ulteriore — in corso di pianificazione da Neurosearch — sarà comunque necessaria se l’Huntexil dovrà diventare un trattamento approvato per la malattia di Huntington.

Vi aggiorneremo sulla sperimentazione non appena i risultati saranno stati annunciati.

Nel frattempo, ci sono farmaci già disponibili, che sono ampiamente usati per aiutare i disturbii del movimento — farmaci come la tetrabenazina, l’olanzapina, il risperidone e il sulpiride.
